All of your passport needs can be met at La Verne City Hall where our one-stop
service also offers passport photos at just $15 per set.

ITEMS YOU WILL NEED TO APPLY FOR A PASSPORT:

• Passport application – completed, but not signed
• Proof of U.S Citizenship – original, long-form (not “abstract”) birth certificate or original naturalization certificate
• Two identical passport photos
• Picture I.D. (i.e. Driver’s license)
• Two blank personal checks for each passport
• Both parents must appear for applicants under age 16 (notarized permission letter or Form DS-3053 may be substituted for one parent)
• All applicants, including children, must be present.
